If you do not agree to these terms in their entirety, please do not use our services. These terms constitute a legally binding agreement between you and Anchor28 LLC.

Description of Services

Anchor28 LLC provides telehealth and in-office healthcare services, including but not limited to medical-assisted weight loss programs, wellness and longevity consultations, peptide therapy, urgent care for non-emergency conditions, and supplemental primary care services such as medication refills and chronic disease management. Our services are provided by licensed healthcare professionals and are subject to the scope of practice regulations in your state of residence.

Eligibility

Our services are available to individuals who are 18 years of age or older and reside in states where Anchor28 LLC is licensed to practice. By using our services, you represent and warrant that you meet these eligibility requirements. We currently serve patients in 43 U.S. states via telehealth and offer in-office visits at our Salem, Oregon location.

Not an Emergency Service

Anchor28 LLC is not an emergency medical service. Our telehealth and in-office services are designed for non-emergency healthcare needs. If you are experiencing a medical emergency — including but not limited to chest pain, difficulty breathing, severe bleeding, stroke symptoms, or any life-threatening condition — please dial 911 immediately or go to your nearest emergency room. Do not rely on our services for emergency care.

Payment and Pricing

Anchor28 LLC operates on a direct-pay model and does not accept insurance. All fees are clearly communicated before services are rendered, and payment is expected at the time of service. Pricing is based on the specific services requested and there are no hidden fees, subscription charges, or unexpected recurring billing. We accept major credit cards and other standard payment methods. Refund policies, if applicable, will be communicated on a case-by-case basis.

Patient Responsibilities

As a patient, you agree to provide accurate and complete information about your medical history, current medications, allergies, and any other health-related information relevant to your care. You understand that withholding or providing inaccurate information may affect the safety and effectiveness of your treatment. You also agree to follow the treatment plan as discussed with your provider and to communicate any concerns, side effects, or changes in your condition promptly.

Intellectual Property

All content on the Anchor28 LLC website — including text, graphics, logos, images, design elements, and software — is the property of Anchor28 LLC or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.
