
HIPAA Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed.
This Notice of Privacy Practices ("Notice") describes how Anchor28 LLC may use and disclose your protected health information (PHI) to carry out treatment, payment, and healthcare operations, and for other purposes that are permitted or required by law. It also describes your rights regarding your health information and how you can exercise those rights. - Right to Access: You have the right to inspect and obtain a copy of your health records maintained by Anchor28 LLC. We may charge a reasonable, cost-based fee for providing copies.
- Right to Request Restrictions: You have the right to request that we limit the uses and disclosures of your PHI. While we will consider your request, we are not required to agree to all restrictions.
- Right to Request Amendments: If you believe that information in your health records is incorrect or incomplete, you have the right to request an amendment. We may deny your request under certain circumstances, but we will provide you with a written explanation.
- Right to Confidential Communications: You have the right to request that we communicate with you about your health matters in a specific way or at a specific location. For example, you may request that we contact you only by mail or at a specific phone number.
- Right to an Accounting of Disclosures: You have the right to receive a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or healthcare operations.
- Right to a Copy of This Notice: You have the right to obtain a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
- Right to Be Notified of a Breach: You have the right to be notified in the event that your unsecured PHI is breached, as defined by HIPAA.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information, provide you with this Notice of our legal duties and privacy practices with respect to your PHI, notify you promptly in the event of a breach of your unsecured PHI, and abide by the terms of this Notice currently in effect. We will not use or disclose your PHI without your written authorization, except as described in this Notice. If you provide us with written authorization, you may revoke that authorization at any time by submitting your revocation in writing.
Changes to This Notice
We reserve the right to change the terms of this Notice at any time. Any revised Notice will be effective for all PHI we maintain at the time of the change, as well as any PHI we receive after the change. We will make the revised Notice available on our website and upon request at our office.
Filing a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Anchor28 LLC or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
