HIPAA Notice of Privacy Practices

This Notice of Privacy Practices ("Notice") describes how Anchor28 LLC may use and disclose your protected health information (PHI) to carry out treatment, payment, and healthcare operations, and for other purposes that are permitted or required by law. It also describes your rights regarding your health information and how you can exercise those rights. Our Responsibilities

We are required by law to maintain the privacy and security of your protected health information, provide you with this Notice of our legal duties and privacy practices with respect to your PHI, notify you promptly in the event of a breach of your unsecured PHI, and abide by the terms of this Notice currently in effect. We will not use or disclose your PHI without your written authorization, except as described in this Notice. If you provide us with written authorization, you may revoke that authorization at any time by submitting your revocation in writing.

Changes to This Notice

We reserve the right to change the terms of this Notice at any time. Any revised Notice will be effective for all PHI we maintain at the time of the change, as well as any PHI we receive after the change. We will make the revised Notice available on our website and upon request at our office.

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with Anchor28 LLC or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
